The initial consultation is $100 and lasts about 90 minutes. Follow up appointments are $80 and last about an hour.

| We are an “out-of-network” provider and do not bill insurance directly. Upon request we will supply you with an itemized receipt that you may submit to your insurance company for reimbursement. Please contact your insurance company for more information regarding your benefits. |

| The office policy requires 24 hours notice for cancellation of an appointment. Failure to give sufficient notice of cancellation will result in a charge of $25. |
